Specialist Referrals

When you need specialty care, we refer you to a trusted network of specialists. We stay in the loop so nothing falls through the cracks.

Referrals That Keep Your Care Connected

Some health problems need a specialist, like a cardiologist, a lung doctor or a surgeon. Your primary care provider decides when that step makes sense and sends you to someone they trust. We share your history, medications and recent labs so the specialist has the context to help you quickly.

The referral is not the end of our role. When the specialist sends back findings, we review them with you and fold them into your plan. Your primary care team stays the center of your care.

  • Referrals to a trusted specialist network
  • Your history, medications and labs sent ahead
  • Specialist findings reviewed with you
  • Ongoing follow-up with your primary care team

Who It Helps

A referral makes sense when a condition needs testing or treatment beyond what primary care offers.

  • Heart or kidney conditions that need a specialist
  • Diabetes complications
  • Lung problems found on a breathing test
  • Joint pain that needs more than injections
  • Symptoms that need a deeper work-up
  • Patients coming home after a hospital stay

How We Coordinate Specialty Care

Good coordination is what keeps a referral from turning into a gap in your care.

The Right Referral

Your provider explains why a specialist is needed and what kind. We handle much in-house first, such as labs, EKGs, breathing tests and joint injections, so you only get referred when it truly helps.

Sharing Your Records

We send the specialist the information they need, including your history, medications and test results. That helps avoid repeat tests and delays.

Reviewing the Findings

When results or recommendations come back, we go over them with you in plain language. Any new medication is checked against what you already take.

Integrated Psychiatry

Our practice includes psychiatry and psychiatric nurse practitioners. When mental health care is part of the picture, it can be coordinated within our own team.

Hospital Coordination

If you are admitted, our team works with the inpatient staff and sees you back in the office within days of discharge.

Frequently Asked Questions

Can I see a specialist without seeing my primary doctor first?

Some insurance plans require a referral from your primary care provider and some do not. Even when it is not required, starting with us means the specialist gets your full history and recent results.

How long does a referral take?

It depends on the specialist's schedule and your insurance. We send referrals with your records so the specialist has what they need to book you.

Do you stay involved after the referral?

Yes. We review the specialist's findings with you, adjust your medications if needed and keep your care plan up to date.

Will my insurance cover the specialist?

Coverage depends on your plan and the specialist's network. Our front desk can help you understand your plan's referral rules before you go.

Can some problems be handled without a referral?

Often, yes. We draw labs, run EKGs and breathing tests, and give certain joint injections in our office, which can save you a specialist visit.
