Nursing Home & Rehab Care in St. Petersburg, FL

For residents of skilled nursing, rehab, assisted living and long-term care facilities, our providers come on-site with the same attention they give in our office.

Care That Comes to the Facility

We partner with skilled nursing centers, assisted living communities, memory care and rehabilitation programs across St. Petersburg and Pinellas County. Our providers make regular rounds, respond when something changes and work closely with facility nurses.

When one of our patients moves into a facility, we keep caring for them there. If a resident later goes home to live with family, we move their care back to our office. If a loved one's facility is not yet a partner, we can often expand coverage on request.

  • Scheduled on-site wellness visits
  • Prompt care when something changes

Frequently Asked Questions

Do your providers visit residents in the facility?

Yes. Our providers make on-site visits to partnered nursing homes, rehab programs, assisted living and long-term care facilities across Pinellas County.

What if my loved one's facility is not on your list?

Call us and ask. If a facility is not yet in our partner network, we are often able to expand coverage on request.

Will my loved one's medicines be reviewed?

Yes. Long-term care residents often end up on more medicines than they need. We review each one regularly and adjust or stop what is no longer helping.

What is the difference between a nursing facility and a skilled nursing facility?

A skilled nursing facility provides daily care from licensed nurses and therapists, often for rehab after a hospital stay. Other nursing and assisted living settings focus more on help with daily living. Our providers see patients in both types of settings.

Can we talk to someone in Spanish?

Yes. Our team serves families in English and Spanish.
